A developer named Antoine did something most people would never think to try: he took the raw DICOM export of his shoulder MRI — a few hundred files, around 266MB — and handed it straight to Claude Opus 4.8 inside Claude Code. Not a chatbot upload. Claude Code is the agentic coding tool, so it wrote its own Python, installed pydicom, parsed the pixel data, and read the scan itself.
Where AI and the radiologist disagreed
The clinic’s report: Grade III partial tear, treatment already started. Claude’s read: tendon intact, only mild tendinopathy. Those aren’t close. When Antoine ran a third arbitration pass, the analysis sided with Claude. He also caught the clinic using shockwave therapy — which guidelines don’t recommend without calcification.
Why this one blew up
It hit HackerNews at 370 points because it’s not a benchmark — it’s a real patient, a real disagreement, and AI winning the tiebreak. Antoine is blunt that you can’t fully trust the model either. But an agent reading raw medical imaging well enough to challenge a specialist is the part nobody can unsee.
